Manhole cover needs maintenance + updating on map

Open reach couldn't find this particular access point on their map as it's not in the correct location as per their system. I'm not exactly sure what runs through there.
I think it needs updating on the various maps based on what utilities run through this point
I also think it needs some maintenance (+ clearance of vegetation). e.g. There seems to be a gap which would allow water in etc.... this doesn't seem ideal and could case future issues.

Land slip - beginning to undermine tarmac
12th May
Flash flooding on 9/5/23 has washed the earth away between the road and river bank. The edge of the tarmac is undermined. Has also exposed the red brickwork making up the drain at the side of the road. Would be wise to repair before further rain causes the road to collapse.
