Uneven surface

When walking from paving slabs to concrete walkway the is a large lip, many of nearby paving slabs broken as well as well as appear subsided. This is very close to bus stop, outside shops. When coming out the shop I tripped causing severe bleeding and a hospital stay 48 hours, transported by ambulance on a blue light to darenth valley hospital at 11.45am on 22nd April.

Hi,
Outside our drive at 4 baldwyn's Park bexley Kent DA52BA are multiple cracked and raised paving slabs that are hazardous to anyone walking or driving on these pavements.
I have raised these pavement issue a couple of times but still hasn't been resolved.
