Large Deep Pothole on the Pavement, 27th March
It has already been reported by our Health & Safety team at Hayley Group Ltd on more than one occasion and it has been marked with paint for repair by Dudley Council officials but has not been repaired.

This is at the entrance to Hayley Group Ltd Site 2 on Shelah Road opposite the oil refinery., 27th March
This pothole has been here for 2 years now and we have had various near miss reports regarding pedestrians tripping on the pavement and also vehicles trying to enter / exit the site and driving on the wrong side to avoid the hole. You got as far as painting around it last year, but it still remains unfilled and an accident waiting to happen. Please can this be made a priority.

Very large deep pothole on pavement into Hayley premises. 44ton lorries, cars, vans, go in and out of this premises daily. Council really needs to fill it in. It's been here for months and months

Several dozen bags of fly tipped rubbish., 20th February
All along the entire length of Avenue Road several dozen black bags of assorted rubbish, household and garden waste are being regularly dumped over the metal railing fence onto what I believe may be land owned by the Railway.

Several vehicles being parked regularly on Double Yellow lines., 20th February
There is a car dealership company 'Compact Car Company' that is using the road clearly marked with double yellow lines in front of the building their business occupies to regularly park their excessive number of vehicles. They are treating the public highway as an extended salesroom and parking several vehicles marked for sale along the public highway on a daily basis. The vehicles are not just parked there while they are moving stock, they are left there all day being offered for sale.

Dangerous manhole in live lane/cycle lane, 10th February
A pothole has formed and exposed a manhole cover in a live lane. It also sits in a cycle lane. Incredibly dangerous especially to cyclists. The manhole cover is now loose and makes a right thumping noise when traffic passes over it.
